I personally think it's between Leafpool or Yellowfang, they both sacrificed a lot, even if they made mistakes as medicine cats, but that's what learning is all about.
Leafpool spent lots of time training Pebbleshine, in fact.. It took moons! Not onlyy that, but she walked in Willowshine's dreams when she was an apprentice and helped her to grow her connection with Starclan! :3

My opinion would probably be Moth Flight, she had kits as a med cat and knew that she couldn't take care of her clan AND her kits, so she and the other OG med cats decided that no med cat in the history of time should take a mate or have kits

Personally, I don’t really like Moth Flight for this exact reason. I think it was unfair of her and starclan to create this rule based on the inability of the FIRST med cat. Of course she’s going to struggle with the care of her kittens and the clan, she has a fraction of what modern medicine cats have. The whole ‘but they would focus on their family first rather than the clan as a whole’ is also a really bad reasoning for the no mate, no kits med rule because that isn’t really the case, and if it were, that mentality would be fixed during their apprentice years under their mentor ( which again Moth Flight didn’t have ). Additionally, med cats have families ( parents, grandparents, littermates )? Why can Leaders have mates and kits when they should also only focus on the clan as a whole and not be biased towards their own, wouldn’t they also face the same inability? Anyways… the point is, I really despise this rule and Moth Flight, not because Moth Flight doing what was best for her which is valid, but making the rule based on her sole experiences was not cool.
