Usually started by the most senior medicine cat (in time spent in the role) they kinda occur whenever in my experience no set time just some time not gathering adjacent as medicine cats (at least when I had snakeheart) would be speaking with eachother then as well. Unless there was a fight which I will admit was common and generally snakes fault in my time.
It didnt occur every moon depending on if people are busy but if you look in starcave there'd be chats herb knowledge sharing in friendly clans or actual herb sharing between allied clans and then those that wished to commune would dream (for space and time the dreams would always kinda occur in a private thread). Some tips I have is if you have a real urge to have a meeting poke the most senior medicine cat for the timing of the next (Its Elmshadow or Cowtuft in your case my memory is foggy) and those not drowning in rps may show up! It doesnt have to be started by them though so its just a good starting point.
If you want to just go without meeting hassle theres nothing to say spring cant go with a warrior that knows the location or drizzle though and starclan dreams can occur anywhere! (Starcave is also a greatplace for cobwebs I used to have snake harvest when the others were asleep so it can be duel purposed)
