Currently you cant apply for deputy and medicine cat/med app. If you scroll to the left you'll see the high ranks, Leader, Deputy, Medicine Cat, and Medicine Cat apprentice. HOWEVER, you can start as kit, apprentice, or warrior anytime in any clan. But make sure your kit ( if you start as one ) has a mother and father.
